Approach and Benefit.

Advanced wound care is recommended for wounds that are slow to heal, worsening, or connected to underlying medical conditions. We commonly treat:

  • Diabetic foot ulcers

  • Venous leg ulcers

  • Pressure injuries (bed sores)

  • Surgical wounds

  • Traumatic injuries

  • Arterial & Venous ulcers

  • Skin Tears
  • Other non-healing wounds

Chronic wounds are often a sign of a larger health concern such as diabetes, vascular disease, infection, or poor circulation. Early evaluation improves healing outcomes and reduces the risk of complications.

If a wound has not improved within two weeks, continues to drain, becomes increasingly painful, or shows discoloration or odor, it should be evaluated promptly.

Successful wound healing depends on more than dressings and procedures. It requires managing the underlying health conditions that affect circulation, oxygen delivery, and immune response.

Through coordinated primary care, we help:

  • Optimize blood sugar levels in diabetic patients

  • Control blood pressure and vascular risk factors

  • Monitor for infection or inflammation

  • Adjust medications that may impact healing

  • Address nutritional deficiencies

  • Coordinate labs, imaging, and specialist referrals

For patients who qualify, we may incorporate Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) to provide additional oversight between visits. This comprehensive approach improves healing and reduces preventable hospitalizations.

Some patients require specialized wound treatment but have difficulty traveling for frequent appointments. Through our partnership with Advanced Mobile Wound Care SC, eligible patients can receive advanced wound services in their home, assisted living facility, or skilled nursing facility.

Mobile wound services may include:

  • Advanced wound debridement

  • Evidence-based dressing selection

  • Infection management

  • Offloading and pressure relief strategies

  • Ongoing wound measurement and documentation

This collaboration ensures patients receive consistent, specialized treatment while remaining under coordinated primary care oversight.

You should seek medical evaluation if a wound:

  • Has not improved after two weeks

  • Becomes red, swollen, or increasingly painful

  • Develops drainage, odor, or discoloration

  • Appears blackened or has dead tissue

  • Occurs in a patient with diabetes or poor circulation

Delaying treatment can increase the risk of infection, hospitalization, or more serious complications. Early intervention leads to better healing outcomes.

In many cases, advanced wound care services are covered under Medicare Part B when deemed medically necessary. Coverage may include wound evaluations, procedures, and ongoing treatment.

For patients enrolled in Chronic Care Management or Remote Patient Monitoring, additional services may also be covered depending on eligibility.

Our team reviews insurance coverage before beginning services and explains any potential out-of-pocket costs so there are no surprises.

If you or a loved one is managing a chronic or non-healing wound, the first step is a comprehensive evaluation. We will assess the wound, review medical history, and determine whether in-clinic care, mobile wound services, or coordinated monitoring is most appropriate.
